A Brief History of the African Nation of Tunisia
Modern Tunisians are the descendants of indigenous Berbers and of people from numerous civilizations that have invaded, migrated to, and been assimilated into the population over the millennia. Recorded history in Tunisia begins with the arrival of Phoenicians, who founded Carthage and other North African settlements in the 8th century B.C. Carthage became a major sea power, clashing with Rome for control of the Mediterranean until it was defeated and captured by the Romans in 146 B.C. Muslim Conquest The Romans ruled and settled in North Africa until the 5th century, when the Roman Empire fell and Tunisia was invaded by European tribes, including the Vandals. The Muslim conquest in the 7th century transformed Tunisia and the make-up of its population, with subsequent waves of migration from around the Arab and Ottoman world, including significant numbers of Spanish Muslims and Jews at the end of the 15th century. From Arab Center to French Protectorate Tunisia became a center of Arab culture and learning and was assimilated into the Turkish Ottoman Empire in the 16th century. It was a French protectorate from 1881 until independence in 1956, and retains close political, economic, and cultural ties with France. Independence for Tunisia Tunisias independence from France in 1956 ended the protectorate established in 1881. President Habib Ali Bourguiba, who had been the leader of the independence movement, declared Tunisia a republic in 1957, ending the nominal rule of the Ottoman Beys. In June 1959, Tunisia adopted a constitution modeled on the French system, which established the basic outline of the highly centralized presidential system that continues today. The military was given a defined defensive role, which excluded participation in politics. A Strong and Healthy Beginning Starting from independence, President Bourguiba placed strong emphasis on economic and social development, especially education, the status of women, and the creation of jobs, policies that continued under Zine El Abidine Ben Alis administration. The result was strong social progress ââ¬â high literacy and school attendance rates, low population growth rates, and relatively low poverty rates ââ¬â and generally steady economic growth. These pragmatic policies have contributed to social and political stability. Bourguiba,à President for Life Progress toward full democracy has been slow. Over the years, President Bourguiba stood unopposed for re-election several times and was named President for Life in 1974 by a constitutional amendment. At the time of independence, the Neo-Destourian Party (later the Parti Socialiste Destourien, PSD or Socialist Destourian Party)ââ¬âenjoying broad support because of its role at the forefront of the independence movementââ¬âbecame the sole legal party. Opposition parties were banned until 1981. Democratic change Under Ben Aliââ¬â¹ When President Ben Ali came to power in 1987, he promised greater democratic openness and respect for human rights, signing a national pact with opposition parties. He oversaw constitutional and legal changes, including abolishing the concept of President for life, the establishment of presidential term limits, and provision for greater opposition party participation in political life. But the ruling party renamed the Rassemblement Constitutionel Dà ©mocratique (RCD or Democratic Constitutional Rally), dominated the political scene due to its historic popularity and the advantage it enjoyed as the ruling party. Survival of a Strong Political Party Ben Ali ran for re-election unopposed in 1989 and 1994. In the multiparty era, he won 99.44% of the vote in 1999 and 94.49% of the vote in 2004. In both elections he faced weak opponents. The RCD won all seats in the Chamber of Deputies in 1989, and won all of the directly elected seats in the 1994, 1999, and 2004 elections. However, constitutional amendments provided for the distribution of additional seats to the opposition parties by 1999 and 2004. Effectively Becoming President for Life A May 2002 referendum approved constitutional changes proposed by Ben Ali that allowed him to run for a fourth term in 2004 (and a fifth, his final, because of age, in 2009), and provided judicial immunity during and after his presidency. As Francisco Dââ¬â¢Anconia tries to make Hank realize this fault, he asks ââ¬Å"Why donââ¬â¢t you hold to the purpose of your life as clearly and rigidly as you hold to the purpose of your mills? Mollââ¬â¢s vanity facilitates her seduction by the elder brother. It is also a strong motif which runs through Mollââ¬â¢s five marriages and numerous lovers. It is a factor which precipitates her decision to steal rather to remain poor and exist only by the honest labor of her needle. In fact, all her actions are in some way linked to her vanity. Mollââ¬â¢s strategies of economical survival are not to be admired upon as everything she has achieved is thanks to lying, cheating, hurting a lot of people and committing crimes. Robinson Crusoe, on the other hand, has completely different ways of surviving. He learns to manage on his own and everything he has accomplished is because of his courage, faith, abilities and amazing survival skills. Both characters are interested in accumulating wealth. Thatââ¬â¢s why Robinson embarks on his journey to take part in trade but in contrast to Moll, he depends only on himself. The heroine in Moll Flanders uses husbands for purely egoistical reasons. She lies about her fortune in order that they agree to be betrothed to her. She manipulates people and accomplishes her devious plan. When sheââ¬â¢s left empty-handed after all of her victims she begins a career of artful thievery using her charm, beauty, wits, femininity, cold-heartedness and evil mind. Then it comes a time when she finally gets the financial security she has always sought but finds her punishment in the Newgate Prison where she gains repentance. Just like Robinson, who deserted on a lonely island finds redemption and a clearer idea of the meaning of life, Moll also changes her point of view and reunites with the one she has most affection for- her ââ¬Å"Lancashire husbandâ⬠. She finds a way to avoid the death sentence and lives freely and happily with her husband. But even finding redemption and re-gaining humanity doesnââ¬â¢t cover the significant difference between her and Robinson. After all the time on that desolate island he retains his pure soul and clear mind. He doesnââ¬â¢t regret anything he has done because he has lived with dignity. He gained prosperity on the island thanks to his strong hands and intelligent mind. Thatââ¬â¢s why Moll, aware of all her sins, canââ¬â¢t be completely happy at the end of the novel. After returning to England together with her husband she says that they are ready to live ââ¬Å"in sincere penitence for the wicked lives we have lived. â⬠(Defoe 296) Unlike Robinson they will spend the rest of their time together contemplating all the wrong paths they have chosen during their robbery days. On the whole, Moll Flanders may be seen as a female version of Robinson Crusoe. Like him, she is obsessed with economic survival and economic success. Unlike him, she prefers to sell her sexual favours or resort to crime- rather than work. The result of the acts she has performed to fulfill her intentions is the transformation of a beautiful innocent young girl into a hardened middle-aged criminal. By illustrating two characters with identical aim but completely different personalities and lifestyles, Defoe makes us think about what is right and what is wrong, how far we would go pursuing our goals and what price we are willing to pay for that.

This means that these people did not believe in separation from the church. Because of this both commonwealths or colonies would probably have churches and laws would come from the church. A difference between these two documents is that the Fundamental Orders of Connecticut is more in depth. The Mayflower Compact only states that a group of people will be going to set up a colony were a government and a church would be set up. The Fundamental Orders of Connecticut says how the government and towns will be run. The Connecticut document says that there will be 2 meetings a year and who will be there and why, and it also states who runs what, and what you must to choose a leader (to vote). Another difference between The Mayflower Compact and The Fundamental Orders Of Connecticut is that the Orders of Connecticut group is setting up a government before they get to Connecticut. Beaches are beautiful forms of nature. Each one varies from another. Some can be rocky, some have white sand, some have black, some are small, and some are large. I love all sorts of beaches. Each beach I have visited has been a memorable experience, but one in particular stands out in my mind. à à à à à During my junior year I went on vacation to a resort in Jamaica with a extraordinary beach. It was a large beach with white sand and remarkable palm trees. There was also a long row of enormous rocks that extended far out into the Caribbean Sea. I found that those rocks made a great place to walk out on to watch the glorious sunsets. à à à à à I thought this Jamaican beach was great for a couple of reasons. One reason was that it was the perfect place to watch the sunset. The other was that it was a great place to lay out relaxing and tanning during the day. Each day, Iââ¬â¢d wake up in the morning, grab a soft white towel, and head for the beach. I was always one of the first to settle on the sand. My Career as a Pharmacist

But why did I choose this career, what is Doctor of pharmacy had to do with my lifetime goal? What will it take me to be successful with my chosen choice of career? With the increasing number of school offering Pharmacy, candidates competing for available seats are just few proofs that the career I have chosen despite its complexity is more than the job with multiple offers. Itââ¬â¢s well paid and well respected in the field of medicine. With the enormous demand for pharmacy professionals one have to consider the rigorous requirements of becoming a Doctor of Pharmacy, a professional degree that needs prior college level of study. For some instances, people who finished a five years pharmacy degree may seek Pharm. D. But this is just one step of becoming a registered pharmacist, there still the preparation and completion of pre-pharmacy requirement, and after that the preparation again for passing the licensure examination. After all of these undertakings, it is now worthwhile that my dreams and goals are materializing, being a part of a medical healthcare team, in touch with people for best drug and non-drug treatment for particular illness, gender and sex, as I can work inside a healthcare facility or being simply a retailer. The continuous modernization in drug therapy that goes with development of technology gives me more excitement because it meant for the increased demand for pharmacist. The work I am competent in, my passion. In this time of modern innovations my career gives me an ongoing enthusiasm for improvement. Accessibility of pharmacies plays a key role in drug distribution during emergencies or simply seeking advice for a particular medicine, prescribing the best drug without its side effects. With the ââ¬Å"war on drugsâ⬠between lawmakers and pharmaceutical company, I can only hope that public will benefit, for cheaper generic prescriptions. There may be growing competitions in this field but the zeal and enthusiasm in me create a positive impact. I know that quality and success donââ¬â¢t happen overnight. Thus, each day I woke up I see to it that it is full of energy and inspiration, because I am meeting different people with different needs on medications and alike. Written Business Communications Effective communication within a business (John Lewis) is essential, in order to ensure their success and profitability.But Firstly, What is Communication? Communication can be defined as ââ¬Å"the exchange and flow of information and ideas from one person to anotherâ⬠[2]. This means that without communication, information or ideas cannot be utilized and no aspect of John Lewis can function properly. For example, if the delivery department cannot communicate with individual stores; therefore this means that John Lewis canââ¬â¢t coordinate with the individual stores, in order to determine what new stock is needed at any particular time.This results in John Lewis being unable to restock their stores properly, meaning products not needed are delivered for example. As a result, John Lewis looses out on potential customers and repeat sales, because consumers who wanted to purchase a specific product couldnââ¬â¢t due to the stock u navailability, leading to a loss in potential sales, repeat sales and profits for the business in the short and long term. He also helped the Saskatchewan Mà ©tis to lead a rebellion against westward expansion. During this time he led t he famous Northwest Rebellion, this lead to his execution by the act of treason. In short, although the impacts upon students are most definitive, such a faulty approach also impacts upon adults due to the fact that encourages each and every member of society to limit and constrain their worldview into a shallow rather presumptuous model of limited understanding. Similarly, another means by which the global citizen and seek to integrate change of a systemic level is with regards to the level and extent to which he/she understands the far-reaching replications of choices and models by which they ascribe to follow. Operant Conditioning involves reinforcement or a reward to maintain a relationship. Dollard and Miller stated that all babies are born with a need to reduce feelings of hunger. When the mother feeds the baby it acts as a positive reinforcement or a reward on the behaviour of the baby. The reward that the baby receives is a positive reward and therefore the baby is likely to repeat the same behaviour to provoke the same response from the mother. This reinforces the attachment bond between mother and child because she is the one providing the food. Dollard Miller (1950) used the term secondary drive hypothesis to describe the processes of learning an attachment through operant and classical conditioning. Secondary drive hypothesis explains how primary drives which are essential for survival, such as eating when hungry, become associated with secondary drives such as emotional closeness. They extended the theory to explain that attachment is a two way process that the caregiver must also learn, and this occurs through negative reinforcement when the caregiver feels pleasure because the infant is no longer distressed.

Learning Case Adaptation :: Technology Case-Based Reasoning Essays
Learning Case Adaptation Computer models of case-based reasoning (CBR) generally guide case adaptation using a fixed set of adaptation rules. A difficult practical problem is how to identify the knowledge required to guide adaptation for particular tasks. Likewise, an open issue for CBR as a cognitive model is how case adaptation knowledge is learned. We describe a new approach to acquiring case adaptation knowledge. In this approach, adaptation problems are initially solved by reasoning from scratch, using abstract rules about structural transformations and general memory search heuristics. Traces of the processing used for successful rule-based adaptation are stored as cases to enable future adaptation to be done by case-based reasoning. When similar adaptation problems are encountered in the future, these adaptation cases provide task- and domain-specific guidance for the case adaptation process. We present the tenets of the approach concerning the relationship between memory search and case adaptation, t he memory search process, and the storage and reuse of cases representing adaptation episodes. These points are discussed in the context of ongoing research on DIAL, a computer model that learns case adaptation knowledge for case-based disaster response planning. 1 Introduction The fundamental principle of case-based reasoning (CBR) for problem-solving is that new problems are addressed by retrieving stored records of prior problem-solving episodes and adapting their solutions to fit new situations. In most case-based reasoning systems, the case adaptation process is guided by fixed case adaptation rules. Practical experience developing CBR systems has shown that it is difficult to establish appropriate case adaptation rules (e.g., Allemang, 1993; Leake, 1994). In defining adaptation rules, a key problem is the classic operationality/generality tradeoff that was first observed in research on explanation-based learning (e.g., Segre, 1987): Specific rules are easy to apply and are reliable, but only apply to a narrow range of adaptation problems; abstract rules span a broad range of potential adaptations but are often hard and expensive to apply because they do not provide task- and domain-specific guidance. In those CBR systems that do perform case adaptatio n, specific rules are often used, requiring that the developer perform difficult analysis of the task and domain to determine which rules will be needed. In practice, the problems of defining adaptation rules are so acute that many CBR applications simply omit case adaptation (e.g., Barletta, 1994). This paper presents a new method by which a case-based reasoning system can learn adaptation knowledge from experience.

Postpartum Endometritis Evidence Based Paper March 13, 2012 Endometritis is the inflammation or irritation of the uterus, which is a common post partum complication that occurs in more than 15% of all pregnancies and is currently the leading cause of maternal mortality (Scott & Hasik, 2001). When endometritis is not related to pregnancy, it is referred to as pelvic inflammatory disease (PID).The Centers for Disease Control and Prevention (CDC) 2010 sexually transmitted diseases treatment guideline defines PID as any combination of endometritis, salpingitis, tubo-ovarian abscess, and pelvic peritonitis. From a pathologic perspective, endometritis can be classified as acute versus chronic. Acute endometritis is distinguished by the presence of neutrophils within the endometrial glands. Chronic endometritis is characterized by the presence of plasma cells and lymphocytes within the endometrial stroma.Chronic endometritis in the postpartum or post abortion patient is usually associat ed with retained products of conception after delivery or abortion. In the nonobstetric population, chronic endometritis has been associated with infections such as chlamydia, tuberculosis, bacterial vaginosis, and the presence of intrauterine devices (Rivlin, 2011). Early-onset postpartum endometritis occurs within two days of delivery, and the late-onset of the disease can occur up to six weeks postpartum.This condition will usually start as a local infection at the placental attachment site and if left untreated, can spread to the entire uterine endometrium (French & Smaill, 2004). There are numerous risks associated with this condition, and diagnosis relies heavily on the clinical judgment of the practitioner. The contamination of the uterine cavity with vaginal organisms during labor and delivery causes the disease. Both bacterial and viral infections may initiate endometritis and many of the agents that cause the infection are naturally present in the vagina.This condition arises commonly after delivery because delivery results in tears, rips or incisions in the vagina, cervix or uterus that allow these agents to enter the uterine lining. The infection can have several species of causative agents that can be aerobic or anaerobic flora (French & Smaill, 2004). The method of delivery will determine which causative agents prognosticate the possibility of endometritis. For vaginal deliveries, the presence of the organisms associated with bacterial vaginosis or genital cultures positive for aerobic gram-negative organisms can indicate endometritis.In cesarean births, the occurrence of certain bacteria such as group A hemolytic streptococci, staphylococci B, Neisseria gonorrhoeae, or Mycoplasma hominis in amniotic fluid cultures will put the patient at an increased risk for this infection (French & Smaill, 2004). With the increasing number of people opting for natural birth methods, including water births, the danger only multiplies. This is becaus e disinfecting procedures as they are carried out before major surgery is usually not practiced in a home environment.Prompt treatment is essential to prevent the spread of the infection through other areas of the body, including the blood. Prolonged infection can be fatal. The immediate postpartum period following birth is a time of increased risk for all women for infection. Microorganisms entering the reproductive tract and migrating into the blood and other parts of the body could result in life threatening septicemia (French & Smaill, 2004). Timely diagnosis and aggressive treatment is essential to prevent these complications.Complications of endometritis include infertility, extension of infection to involve the peritoneal cavity with peritonitis, intra-abdominal abscess, and septic pelvic thrombophelbitis. Septic pelvic thrombophelbitis is a condition in which blood clots in one of the pelvic vessels become infected. If untreated it could progress to septic pulmonary embo li, in which the infected blood clots travel to the lungs and lead to death (French & Smaill, 2004).Septic shock is a life-threatening systemic infection usually caused by bacteria and on rare occasions follows postpartum endometritis. The bacteria that invade the bloodstream release a substance known as endotoxin, which causes decreased blood pressure, clot formation, major tissue injury, and leakage of fluids. Accordingly, organs may fail because they are not receiving enough blood and nutrients. Fortunately, this condition during pregnancy or in the postpartum period is a rare clinical event (Mazzeffi and Chen, 2010).Major risk factors for obstetric endometritis include the following: Cesarean delivery (especially if before 28 weeks' gestation), rupture of membranes lasting more than 24 hours, excessively long labors, severely meconium-stained amniotic fluid, manual placental removal, and extremes of patient age. Other threatening factors have been identified in additional st udies, including no prior cesarean delivery, preterm or postterm gestation, low infant Apgar scores, antepartum infections, preeclampsia, amnioinfusion, postpartum anemia, the presence of internal monitors, and steroid medications (Olsen, Butler, Willers ;amp; Gilad, 2010).Acute endometritis is typified by the existence of neutrophil cells in the endometrium. Neutrophils are white blood cells with cytoplasmic granules that consume harmful bacteria, fungi, and other foreign materials. Characteristic symptoms of endometritis include abdominal distention or swelling, abnormal vaginal bleeding, abnormal vaginal discharge, fever (100 to 104 degrees Fahrenheit), general discomfort, uneasiness, or ill feeling (malaise), and lower abdominal or pelvic pain (uterine pain). Anemia occurs when a patientââ¬â¢s red blood cell count is lower than 4. -6. 0 million red blood cells per micro liter of blood. Losing large amounts of blood during delivery or prior to delivery may be a contributing fa ctor for a low red blood count, anemia and potentially endometritis. Red blood counts (RBC) are needed to indicate anemia and the sedimentation rate (ESR). The sedimentation rate measures the rate at which red blood cells sediment in a period of 1 hour. It is a common hematology test that is a non-specific measure of inflammation, which is evident in endometritis.The diagnosis of postpartum endometritis is based on the presence of fever in the absence of any other cause. Uterine tenderness, purulent or foul-smelling lochia and leukocytosis are common clinical findings used to support the diagnosis of endometritis. Leukocytosis is a raised white blood cell count (the leukocyte count) above the normal range in the blood. The standard definition for puerperal fever used for reporting rates of puerperal morbidity is an oral temperature of 100. 4 degrees centigrade or more on any two of the first ten days postpartum or 101. degrees or higher during the first 24 hours postpartum (French ; amp; Smaill, 2004). Additionally, when the above symptoms occur, urinalysis and urine culture may be done. However, endometrial cultures are rarely indicated because specimens collected through the cervix are usually contaminated by vaginal and cervical flora. A sterile technique with a speculum is used to avoid vaginal contamination, and the sample is sent for aerobic and anaerobic cultures. If fever persists for 48 hours (Some clinicians use a 72-hour cutoff) after endometritis is adequately treated, ther causes such as pelvic abscess and pelvic thrombophlebitis should be considered. Abdominal and pelvic imaging, usually done by CT, is sensitive for abscess but detects pelvic thrombophlebitis only if the clots are large. If the results of the imaging are negative, a trial of heparin is typically begun to treat presumed pelvic thrombophlebitis as a diagnosis of exclusion (Moldenhauer, 2008). Before the advent of the antibiotic era, puerperal fever was an important cause of maternal death.With the use of antibiotics, a sharp decrease in maternal acute postpartum infections has been observed, and it is now accepted that antibiotic treatment for postpartum endometritis is warranted. Intravenous broad-spectrum therapy (cephalsporins, penicillins, or clindamycin and genatmicin) is appropriate for the treatment of endometritis. Regimens with activity against penicillin-resistant anaerobic bacteria are better than those without. There is no evidence that any one regimen is associated with fewer side effects.Once uncomplicated, endometritis has clinically improved with intravenous therapy, and oral therapy is not needed (French, 2003). Furthermore, it is essential that the patient receive supportive care including hydration, rest and pain relief. Antibiotics should be discontinued 24 hours after the patient is asymptomatic. Assessments should be taken of the lochia, vital signs, and changes in the womenââ¬â¢s condition continue during treatment (Perry, Hockenberry ;amp; Lowdermilk, 2010). Treatment is usually considered successful after the woman is afebrile for 24 to 48 hours.If the initial antibiotic regimen does not result in resolution of fever and other symptoms within three days, the antibiotic regimen is usually changed. Consideration is also given to the possibility that the woman may have complications requiring specific treatment. The most effective treatment and least expensive treatment of postpartum infection is prevention. Preventative measures include good prenatal nutrition to control anemia and intrapartal hemorrhage. Good maternal perineal hygiene with through hand hygiene is emphasized.Strict adherence to aseptic techniques by all healthcare professionals during childbirth and the postpartum period is very important (Perry, Hockenberry ;amp; Lowdermilk, 2010). Endometritis is usually caused, in the postpartum scenario, because of a deficient care taken to avoid streptococcus and staphylococcus infections in the delivery ar ea. These two bacteria are present on every inch of our skin, and considering that delivery is the one time when the mother's insides are most exposed, precautionary measures to maintain a sterile environment in the delivery or birthing room should be taken.The benefit of antibiotic therapy for laboring women has been unquestionably established. Intravaginal metronidazole as surgical preparation and oral methylergometrine after delivery are two interventions that show promise as additional prophylactic interventions (French, 2003). Having a baby by Caesarean section is becoming increasingly common, despite the higher risks associated with the surgery compared to a vaginal birth. One important concern is the risk of infection, which is between five and 20 times greater for women who undergo scheduled or emergency Caesarean section.According to the Cochrane Review, ââ¬Å"the single most important risk factor for postpartum maternal infection is Caesarean section. â⬠The review f urther cited that antibiotics to women undergoing Caesarean section reduced the incidence of fever by 45 percent, wound infection by 39 percent, inflammation of the uterine lining by 38 percent and serious infectious complications for the mother by 31 percent (Nelson, 2010). This approach can significantly lower the risk of endometritis, particularly in women having surgery after extended labor and ruptured membranes.To prevent future infection, most doctors prescribe Cefazolin, which is administered intravenously immediately after the baby's umbilical cord is clamped. If you are at high risk, a second dose may be given eight hours later (French ;amp; Smaill, 2004). The overall goal for the postpartum client with endometritis is, ââ¬Å"The patient will be free from infection. â⬠Nursing management and general interventions of the patient would include the collection of vaginal and blood cultures, education on handy hygiene, the administration IV antibiotics and analgesics as p rescribed.Non-pharmacological interventions include distraction, imagery, relaxation, and application of hot and cold. Non-pharmacological interventions can restore the clientââ¬â¢s sense of self-control, personal efficacy, and active participation in her care. It is essential that the information and method of delivery of information be tailored to the specific client and family (French ;amp; Smaill, 2004). Secondary to free from infection, an accurate nursing care plan for a postpartum patient with an with endometritis would include: 1.The patient will follow a specific, mutually agreed upon, healthcare maintenance plan. (The nurse should assume that first-time mothers lack sufficient knowledge regarding condition and treatment diagnosis, and therefore, needs education and specific instructions during the postpartum recovery period). If a mother has given birth to more than one child a review of proper heath care regimens is also justifiable. The new mother should receive instr uction pertaining to hygienic care for her perineal area. This care would include changing her perineal pad frequently and washing her hands afterwards.The presence of a wet pad against sutures is an excellent medium for the development of an infection that could potentially spread to the uterus. The use tampons should be prohibited for six weeks after delivery, since tampon use can cause infection or even toxic shock syndrome. It is the nurseââ¬â¢s responsibility to promote adequate rest and encourage a generous intake of nutrients and fluids. The patient will report that pain management regimens achieves comfort function goal without adverse effects (Ackley ;amp; Ladwig, 2011).The nurse should administer comfort measures to ease pain and teach the patient proper understanding of the condition as well as taking measures to correct the complications of endometritis (Perry, Hockenberry ;amp; Lowdermilk, 2010). 2. The patient will maintain oral temperature within adaptive levels (l ess than 100. 4 degrees). Evaluate the woman's temperature at the end of the first hour postpartum and then every four hours for the first 22 hours postpartum. Clients with endometritis typically have a fever, chills, general malaise, and may exhibit tachycardia.Oral temperature measurement provides a more accurate temperature than tympanic measurement, auxiliary, or use of a chemical dot thermometer. Despite the normalcy of childbirth, complications may arise that will have detrimental effects on the postpartum client. These include postpartum hemorrhage, thrombophlebitis, and infections such as endometritis. Healthcare providers working with postpartum clients must have a clear understanding of these complications, including the symptoms, nursing interventions, and treatment.A cognizant nurse would carefully review the results of laboratory tests for signs of anemia, infection, and electrolyte imbalance. Blood cultures to identify the causative agents of potential infections are typically done, and white blood cell (WBC) counts are monitored. However, it is important to remember that the white blood cell count is normally elevated after delivery for a short period; continued monitoring of the WBC count is required in identifying endometritis (French, L. , & Smaill, F. M, 2004).Nearly 90% of women treated with an approved regimen note improvement in 48-72 hours. Del ay in initiation of antibiotic therapy can result in systemic toxicity. Endometritis is associated with increased maternal mortality due to septic shock. However, mortality is rare in the United States because of aggressive antimicrobial management. Most cases of endometritis, including those following cesarean delivery, should be treated in an inpatient setting.
